WIP: Index sdii (closes #7)

If index_function sdii [class SDII] is only to be used for index sdii then neither reducer nor threshold is needed. It involves a statistic based on data at or above a threshold, thus is does not fit perfectly with the other indices in the simple stats group, as per the preliminary grouping
